$20 USD permit fee
A photocopy of both sides of your driver's license
Then simply take these items to your local AAA office (or submit them by mail, depending on where you live).

UPDATE: During our visit to Grand Cayman in June 2019, we learned that they no longer require the International Driving Permit, and they no longer require that you buy a Cayman Islands Temporary Driving Permit.
